What do you see?

The Make Report Selection screen allows you to choose the reports you want your provider to include with your monthly bill.  Where appropriate, this screen also allows you to select the criteria on which the selected reports will be based.

• Fields and buttons

  Field	Description
  PAC Summary	Select this check box to include the PAC Summary  report with your monthly bill.
  Total Usage	Select this check box to include the Total Usage report with your monthly bill.
  High Call Volume	Select this check box to include the High Call Volume report with your monthly bill.
  HC Unit of Measure	This drop-down list allows you to select the value on which the High Call Volume report will be based.
  Threshold Value	This field allows you to type the value that determines "high volume".  The High Call Volume report will include all values higher than the number you type here.   Note:  This value must be between 5 and 999.
  Long Duration	Select this check box to include the Long Duration report with your monthly bill.
  LD Unit of Measure	This drop-down list allows you to select the value on which the Long Duration report will be based.
  Threshold Value	This value allows you to type the value that determines "long duration".  The Long Duration report will include all values higher than the number you call here.   Note:  This value must be between 30 an 999.
  Unanswered Call	Select this check box to include the Unanswered Call report with your monthly bill.
  Top City	Select this check box to include the Top City report with your monthly bill.
  Threshold Value	This field allows you to type the value that will determine the top city in the report.   Note:  This value must be between 5 and 100.
  Area Code Summary	Select this check box to include the Area Code Summary report with your monthly bill.
  Sort Order	This drop-down list allows you to choose how the Area Code Summary report will be sorted.
  LATA Summary	Select this check box to include the LATA Summary report with your monthly bill.
  	Click this button to return to the last screen you were viewing.
  	Click this button to continue the process of selecting the reports to be included with your monthly bill.  When you click this button, the system takes you to the View and Save Report Selections screen.
  	Click this button to clear all the check boxes and fields on this screen so you can start over.
  	Click this button to end the process of selecting the reports to be included with your monthly bill without taking any action.  When you click this button, the system takes you back to the Choose Options screen.
