What do you see?

The Select the path to take screen allows you to access help with choosing the reports that will be included with your monthly bill.

• Fields and buttons

  Field	Description
  	Click this button for help in completing the report request.  When you click this button, the system takes you to the Get Instructions screen.
  	Click this button to see the number for Qwest's Customer Service.  When you click this button, the system displays a toll-free number you can call to get further assistance.
  	Click this button to continue the process of selecting the reports to be included with your monthly bill.  When you click this button, the system takes you to the Make Report Selection screen.
  	Click this button to end the process of selecting the reports to be included with your monthly bill.  When you click this button, the system returns you to the Choose Options screen.
