The Qwest Terms and Conditions screen allows you to complete the process of placing an order.  On this screen, you review the terms and conditions of placing the order, and review the order information.  This screen is divided into several sections.  Click below for more information.

• Fields and buttons - Qwest Terms and Conditions

  Field	Description
  Qwest Terms and Conditions	This text box displays all the terms to which you must agree in order to place your order.  Review this information carefully before you place the order!
  I Agree	If the Terms and Conditions are acceptable to you, select this check box.   Note:  If any portion of the Terms and Conditions is not acceptable to you, leave this check box blank and click .

   

• Fields and buttons - About This Order Request

  Field	Description
  Total Charges on this Order Request	This field displays the total amount of money(MRC + NRC + Miscellaneous Charges) that will be billed for this order.
  Total MRC	This field displays the total dollar amount of the Monthly Recurring Charges for this order.
  Total NRC	This field displays the total dollar amount of the Non-Recurring Charges for this order.
  Total Miscellaneous Charges	This field displays the total dollar amount of any additional charges for this order.
  Enterprise ID	This field displays the unique identifier for the customer Enterprise for which this order is being placed.
  Letter of Agreement	This field allows you to access a printable version of the Letter of Agency you must sign and return before the order can be completed.   Note:  This document is presented as a PDF(Portable Document Format) file.  You will need at least the Adobe Acrobat Reader in order to open the document.  If you do not have the Reader, you can download it for free here.

   

• Fields and buttons - Customer Contact (Purchasing Agent) Information

  Field	Description
  Contact Name	This field displays the name of the person responsible for making purchases for the company placing the order.
  Contact Type	This field indicates the Contact's role within the company placing the order.
  Company Name	This field displays the identifier for the company placing the order.
  Location	This field displays complete address at which the purchasing contact can be reached.
  Phone	This field displays the telephone number at which the purchasing contact can be reached.
  Facsimile	This field displays the telephone number to which FAXes to the purchasing contact can be sent.
  eMail	This field displays the full email address at which the purchasing contact can be reached.
  Pager	This field displays the pager number at which the purchasing contact can be reached.
  Cell Phone	This field displays the cell phone number at which the purchasing contact can be reached.

   

• Fields and buttons - Progress Status

  Field	Description
  Worksheet	This field identifies the total number of items on the current Worksheet.
  Shopping Cart	This field displays the total number of items that are in currently in the Shopping Cart associated with the current Worksheet.

   

• Fields and buttons - Worksheet Items Section

  The List of TN#s section allows you to select the telephone number(s) with which you want to associate the selected feature(s).

  Field	Description
  	These checkboxes allow you to choose the telephone numbers you want to associate with the selected location.
  TN#	This column lists all the telephone numbers available for the location you selected.
  Billing Account	This column displays the unique identifier for the billing account associated with each listed telephone number.  This value is a link.  When you click this link, the system opens a new browser window, displaying the Billing Account Details screen.
  Service Address	This column displays the full address with which each listed telephone number is associated.  This value is a link.  When you click this link, the system displays more information about the selected address.
  IntraLATA	This field identifies the long distance carrier that handles IntraLATA(IntraLATA telecommunications services originate and end in the same Local Access and Transport Area, or LATA.) traffic for the selected telephone number.
  InterLATA	This field identifies the long distance carrier that handles InterLATA(InterLATA telecommunications services originate in one Local Access and Transport Area, or LATA, and end in another.) traffic for the selected telephone number.
  Feature	These columns identify all the features associated with the selected telephone number.  You can click this value to see a definition of the selected feature.
  Status	These columns display the current status of each listed feature.
  	Click this button to end the ordering process without taking any actions.  When you click this button, the system takes you to the last screen you were viewing.
  	Click this button to submit your order.  When you select the I Agree check box and click this button, the system attempts to place your order, then takes you to the Order Request Confirmation screen.

What do you want to do?

After reviewing the information on this screen, select the I Agree check box, then click .  The system will attempt to submit your order, then take you to the Order Request Confirmation screen.  No other tasks are associated with this screen.

Note:  You can end the ordering process without taking any action by clicking .  The system will return you to the last screen you were viewing without taking any action.
