What do you see?

The Create Ticket screen allows you to enter a repair ticket for any component of your services that may be experiencing a problem.

• Fields and buttons

  Product and Problem Information Section
  Field	Description
  Product	This drop-down list requires you to select product experiencing the problem you want to report from all the products and services associated with your Enterprise.
  Product Component Type	This box requires you to choose the kind of service component experiencing the problem you want to report.   Note:  This box only appears if you select DIA, DWH, LD, or MFWVPN from the Product drop-down list.
  Component ID	This field requires you to indicate the specific service component that is experiencing the problem.
  	This button requires you to select the service component experiencing the problem from a list of the service components associated with your Enterprise.
  Problem/ Component Type	This list requires you to indicate the problem being experienced by the selected service component.
  Symptom Type	This list requires you to select a general category for the symptom your service component is experiencing.
  Problem Description	This field requires you to type a more detailed description of the trouble your component is experiencing.
  Primary Contact Information Section
  Field	Description
  First Name	This field requires you to type the first name of the main person your provider can contact for more information, if necessary.
  Last Name	This field requires you to type the surname of the main person your provider can contact for more information, if necessary.
  Phone	This field requires you to type the telephone number at which your provider can reach the primary contact for more information, if necessary.
  Ext	This field allows you to type a telephone extension for the primary contact, if needed.
  Email	This field requires you to type the e-mail address at which your provider can reach the primary contact for more information, if necessary.
  Secondary Contact Information Section
  Field	Description
  First Name	This field allows you to type the first name of another person your provider can contact for more information, if necessary.
  Last Name	This field allows you to type the surname of the second person your provider can contact for more information, if necessary.
  Phone	This field allows you to type the telephone number at which your provider can reach the secondary contact, if necessary.
  Ext	This field allows you to type a telephone extension for the primary contact, if needed.
  Email	This field allows you to type the e-mail address at which your provider can reach the secondary contact, if necessary.
  	Click this button to begin the process of entering changes you want made to a selected Firewall.  When you click this button, the system takes you to the Firewall Change page.
  	Click this button to send the repair ticket to your provider. The system will display a confirmation screen to confirm the information you entered.
  	Click this button to exit the Create Ticket screen without saving anything.
