What do you see?

The Unenroll From Auto Payment screen allows you to provide confirmation and authorization for removing Auto Pay from the selected account(s).  This screen identifies the account(s) you selected for removal, and prompts you for authorization for the removal.

• Fields and buttons

  Field	Description
  Account #	This column contains the unique, provider-maintained identifier for each account you selected.
  Account Name	This column contains the customer-maintained identifier for each account you selected.
  Type	This column identifies the kind of account each listed account is.  Values you will see include:   QLD(Qwest Long Distance)   QLS(Qwest Local Service)
  Balance	This column displays the total outstanding amount owed for each listed account.
  I Authorize Qwest...	This check box allows you to confirm that you do, in fact, want to remove Auto Pay from the selected account(s).   Note:  This check box is your authorization for your provider to remove the Auto Pay feature.  If you do not select this box, your provider cannot remove Auto Pay, and will continue to debit your account for your monthly balance.
  Name	This field allows you to type your name for authorization purposes.   Note:  The date on which you fill out this screen appears after this field.
  	Click this button to return to the last screen you visited.
  	Click this button to submit your authorization for your provider to cancel Auto Pay.
  	Click this button to end the process of removing Auto Pay without taking any action.  When you click this button, the system returns you to the Pay Bills screen.
