What do you see?

The Confirm Your Paperless Unenrollment screen allows you to review the account(s) you selected to remove from the Paperless Billing delivery option.  In this screen, you authorize the cancellation.

This screen is divided into two sections.  The Unenroll From Paperless section lists the provider accounts you selected in the Unenroll from Paperless Billing screen.  The Authorization section allows you to provide final authorization for your provider to end your paperless bill delivery.

  Selected Accounts Section
  Field	Description
  Invoice Delivery Method	This field confirms that the accounts you have selected are signed up for paperless delivery.
  Account #	This column displays the unique, provider-maintained identifier for each account you selected to remove from paperless billing.
  Account Name	This column displays the unique, customer-maintained identifier for each account you selected to remove from paperless billing.
  Type	This column identifies whether each listed account is QLS(Qwest Local Service) or QLD(Qwest Long Distance).
  Balance	This column displays the current balance due for each listed account.
  Authorization Section
  Field	Description
  I authorize...	Select this check box to confirm that you do, indeed, want to remove the selected accounts from paperless bill delivery.
  Name	This field allows you to provide an electronic signature confirming that you authorize the removal of paperless billing from the selected accounts.
  	Click this button to return to the last screen you were viewing if you need to make corrections.
  	Click this button to complete your enrollment in automatic payment.  When you click this button, the system takes you to the Paperless Unenrollment Complete screen.
  	Click this button to end the process for making a payment by bank account without taking any action.  When you click this button, the system returns you to the Choose Options screen.
