What do you see?

The Schedule Appointment - Select Service Address screen allows you to choose the address for which you want to schedule an appointment to install one or more new telephone numbers.  This screen is divided into a number of sections.  Click below to see more information on each section.

• Fields and buttons - Tasks & Assistance Section

  The Tasks & Assistance section helps you work through the order.

  Field	Description
  Current Task	This field briefly describes what you should be doing on the current screen.
  Next Step	This field explains what you need to do next in order to complete your order.
  Do You Need Assistance?	These radio buttons allow you to request more help with the current step in the ordering process.  When you click the Yes radio button, additional instructions appear on the screen.

   

• Fields and buttons - Service Address Selection Section

  The Service Address Selection section allows you to select the location for which you want to schedule an installation, and to indicate whether new jacks will be required.

  Field	Description
  Select Existing Service Address	This drop-down list allows you to select the address for which you want to schedule the installation.
  Install Jack?	These radio buttons allow you to indicate whether or not the location requires jacks to be installed for the new telephone number(s).

   

• Fields and buttons - Worksheet Items Section

  Field	Description
  Jack Quantity	The fields in this column allow you to type the number of telephone jacks that need to be installed for each listed telephone number.   Note:  These fields are only available if you select the Yes radio button from the Install Jack? field above.
  TN#	This column displays the telephone numbers to be installed at the selected location.
  Installation Appointment	This column displays the date and time on which each listed telephone number is set to be installed.
  Billing Account	This column displays the unique identifier for the Billing Account associated with each listed telephone number.
  Service Address	This column displays the physical address associated with each listed telephone number.
  Select an Action	This drop-down list allows you to select the next action you want to take.  You can choose to view installation appointments that are available for the selected address, or to cancel the appointment scheduling and return to the previous screen.
  	Click this button to take the action you selected above.  If you chose to select an installation appointment, the system will take you to the Schedule Appointment - Select Available Appointment screen when you click this button.  If you chose to cancel scheduling the appointment, the system will take you to the last screen you were viewing when you click this button.
