What do you see?

The Schedule Appointment - Select Available Appointment screen allows you to choose the date and time on which you want the new telephone number to be installed.  This screen is divided into a number of sections.  Click below to see more information on each section.

• Fields and buttons - Tasks & Assistance Section

  The Tasks & Assistance section helps you work through the order.

  Field	Description
  Current Task	This field briefly describes what you should be doing on the current screen.
  Next Step	This field explains what you need to do next in order to complete your order.
  Do You Need Assistance?	These radio buttons allow you to request more help with the current step in the ordering process.  When you click the Yes radio button, additional instructions appear on the screen.

   

• Fields and buttons - Installation Appointment Details Section

  The Installation Appointment Details section allows you to select the date and time on which you want the installation to take place, and to provide additional information for the installer.

  Field	Description
  Select available appointment	This drop-down list allows you to select the date and time on which you want the installation to occur.  The list contains all appointments that are still available at the current time.
  Comments	This text box allows you to provide additional information for the installer.  You can use this space to provide access information, such as "call first" or "go to the side door".
  First Name	This field requires you to type the first name of the person we should contact about the installation.
  Last Name	This field requires you to type the last name of the person we should contact about the installation.
  Tele No	This field requires you to type the complete telephone number at which we can reach the contact you named above.
  Pager	This field allows you to provide a telephone number at which we can page the contact you named.

   

• Fields and buttons - Worksheet Items Section

  Field	Description
  Jack Quantity	This column identifies the number of jacks that need to be installed for each listed telephone number.
  TN#	This column displays the telephone numbers to be installed at the selected location.
  Installation Appointment	This column displays the date on which each listed telephone number is set to be installed.
  Billing Account	This column displays the unique identifier for the Billing Account associated with each listed telephone number.
  Service Address	This column displays the physical address associated with each listed telephone number.
  Select an Action	This drop-down list allows you to select the next action you want to take.  You can choose to schedule the date and time you selected above, or to return to the Schedule Appointment - Select Service Address screen without taking any action.
  	Click this button to take the action you selected above.  If you chose to schedule the installation appointment, the system takes you to the appropriate screen to continue the process.  If you chose to go back, the system returns you to the Schedule Appointment - Select Service Address screen without taking any action.
