What do you see?

The Confirm Your Automatic Payment ACH Enrollment Information screen allows you to review the information you provided for accounts you want to set up for automatic payment.  In this screen, you authorize your provider and your financial institution to pay your account balances automatically from the account you selected.

This screen is divided into three sections.  The Account Information section displays the bank account information you provided on the Enroll in Automatic Payment Bank Account Information screen.  The Selected Accounts section lists the accounts you selected in the Enroll in Automatic Payment screen.  The Authorization section displays the terms and conditions(Review this information carefully to make sure you fully understand before you provide final authorization.) that apply to automatic payments.  This section also allows you to provide final authorization for your provider and your financial institution to pay your bills automatically.

• Fields and buttons

  Account Information Section
  Field	Description
  Payment Method	This field confirms that you are setting up accounts for Automatic Payment ACH(Automated Clearing House).
  Routing Number	This field displays the 9-digit account routing number you entered for the bank account from which funds will be withdrawn.
  Account Number	This field displays the unique identifier you entered for the bank account from which funds will be withdrawn.  This value can be up to 22 digits in length.
  Name on Account	This field displays the name you entered as the owner of the bank account from which funds will be withdrawn.
  Selected Accounts Section
  Field	Description
  Account #	This column displays the unique, provider-maintained identifier for each account you selected to sign up for automatic payment.
  Account Name	This column displays the unique, customer-maintained identifier for each account you selected to sign up for automatic payment.
  Type	This column identifies whether each listed account is QLS(Qwest Local Service) or QLD(Qwest Long Distance).
  Balance	This column displays the current balance due for each listed account.
  Authorization Section
  Field	Description
  I authorize...	Select this check box to confirm that you do, indeed, authorize your provider to withdraw funds automatically from the account you identified.
  Name	This field allows you to provide an electronic signature confirming that you have read and agree to the authorization and the terms and conditions of the automatic payment.
  	Click this button to return to the last screen you were viewing if you need to make corrections.
  	Click this button to complete your enrollment in automatic payment.  When you click this button, the system takes you to the ACH Enrollment Complete screen.
  	Click this button to end the process for making a payment by bank account without taking any action.  When you click this button, the system returns you to the Pay Bills screen.
